I’ve recently gotten a few questions about document hierarchy. Since I haven’t yet created the master document for any Management System (The Management System Manual), I was unable to easily answer the questions.
Here’s a quick summary …

The management system is made up of policies (PL), operating procedures (OP), work instructions (WI), forms (F), etc. organized under the elements of the system:
Series 1000 – Management System Manual
Series 4000 – Management System Administration
Series 5000 – Management Responsibility
Series 6000 – Human Resources
Series 7000 – Execution
Series 8000 – Measurement, Analysis, and Improvement
So, the first operating procedure under Human Resources would be OP6001. If it has a form that is used as part of the process (and many do) the first form would be OP6001-F001. The second form attached to that OP would be OP6001-F002 … and so on.
The first policy under management responsibility would be PL5001. The first operating procedure would be OP5001.
Work instructions are always connected to operating procedures. So the first work instruction under the first operating procedure in the execution area would be OP7001-WI001.
